In-Home Evaluation · $475: the same evaluation in the comfort of home.

Re-Evaluation · $250: for returning clients, recommended every 6 to 12 months when a condition is progressive.

Family Consultation · $150: planning for driving changes and transportation alternatives, without testing.

Private pay. A superbill is available on request. Evaluations are clinic-based and do not include on-road testing.
